Chronic Low-Back Pain

FIGURE 4.15. Back pain is one of the most common pain disorders, with 70-85% of all people having experienced back pain at some point in their lives. Obesity and tobacco usage are strong predisposing factors for lower back pain.

Chronic low-back pain, like osteoarthritis, does not derive from neuronal damage, but rather from spine or disc problems in the back. Central sensitization, however, can exacerbate this disorder, making non-opiate pain treatments helpful and often necessary.

Studies of pharmacological efficacy of norepinephrine versus serotonin reuptake inhibitors reveal that noradrenergic mechanisms are more effective at mitigating chronic low-back pain, particularly low doses of drugs with norepinephrine reuptake-inhibiting properties.
